Newcomers in Ontario are eligible for the Ontario Health Insurance Plan (OHIP). You can apply for OHIP at any Service Ontario office, which are available in every city or township. You will need to complete the Registration for Ontario Health Coverage form:

You will also need to bring:

1. Proof that you are lawfully admitted to Canada (a permanent residency card)

2. Proof that you live in Ontario (Ontario driver’s license, income tax assessment)

3. An additional document supporting your identity (i.e., a passport from any country, a credit card, etc.).
